WE ACCEPT THE FOLLOWING

A referral form and risk assessment from the placing authority or a completed Winners of Tomorrow Support Services referral form and risk assessment is accepted.
We will consider emergency placements at the Unit. In such circumstance (if out of hours) then telephone information is accepted on the proviso that a referral form will follow within 12 hours of admission. All other emergency placements will require a completed referral form and, additional information if required and a completed risk assessment.
We will undertake a Signs of Safety Risk Assessment on all referrals to ensure the wellbeing of all service users also, to determine whether the unit is suited to meet the needs of the young person.

PREPARATION FOR THE YOUNG PERSON

STEP 2

The allocated keyworker will be identified in preparation for the young person. Wherever possible the Keyworker will meet the young person upon arrival as this is the start in establishing a consistent and positive working relationship.

STEP 3

All young people will receive a welcome/starter kit upon arrival.
Within 7 days of residence a meeting will be organised with Winners of Tomorrow management, the Young Person and Social Worker to discuss the young person’s specific needs and how best to meet them.

STEP 4

A Key-working meeting will take place within 48 hours from placement (except in exceptional circumstances), this will feed into the young person’s Pathway Plan.
